I. Physical Ability I, the parent/guardian of the child registered to play a sport for Sacred Heart am providing notice that my child is physically able to participate in this activity and as such it is my responsibility to bring any health or physical conditions that affect my child to the attention of his or her coach.

II. League Formation I understand that the Sacred Heart Athletic Directors have the sole jurisdiction of placing and transferring players from team to team for the sake of establishing teams.

III. Commitment
I understand that participating in CYO sports requires commitment and dedication to the team. I accept that the coaches will make all decisions with regards to playing time, player position, plays & tactics, etc.  My child will attend all practices and games and any other team events and will notify the coach in a timely manner regarding lateness or absence. I understand that absence or consistent lateness from practice may negatively impact playing time.

V. Conduct I agree to be encouraging, supportive, and positive with regard to my child's play on the field.
I agree to support the coach and the team and not interfere at any time with the duties and responsibilities of the coach.
I agree to familiarize myself with the rules of the games and respect officials and accept their decisions.
I understand that it is my responsibility to act in a manner appropriate for a spectator of a Catholic Youth Organization event. I agree to not engage in any kind of unsportsmanlike conduct with any official, coach, player or parent and that behavior unbecoming of a parent or fan representing Sacred Heart will be subject to suspension from attending future events.
I am also responsible for the behavior of any friend or family member who attends CYO events at Sacred Heart and other hosting locations.

VI. Gym Safety I understand that it is everyone's responsibility to maintain a safe environment at the Sacred Heart Gym. During practice sessions, I will not prop the doors open and will use best judgement when permitting others to enter the building.

VII. Communication I agree to receive messages related to Sacred Heart Youth Program via text message, e-mail, and phone. I understand that there may be photos, videos and web site and social media posts that include the players and teams.

VIII. Payments Full payment is due at the time of registration. There will be no refunds due to inability to comply with these guidelines and policies, schedule conflicts or for any other reason.  In addition to registration fees, there are entrance fees to the gym during competitions and there may be optional tournaments that the coach enters a team that may require additional entry fees.

IX. Eligibility I agree that I have been made aware of the CYO eligibility requirements and my child meets the eligibility status that is indicated on this form and that all information on this form is 100% accurate, especially date of birth and address information. I understand that at any point throughout the season I may be asked to show proof of age or address and incorrect information may result in suspension without refund.
